[4eyes] Thesis LaTex Template

I've used this:
http://www.movesinstitute.org/~kolsch/ucthesis/ucthesis.html . The
only thing the grad division requested me to change was to write
"Professor" or "Doctor" on the cover page instead of "Prof." or "Dr."
